The Career Transition Program (CTP) has a fifteen year track record in Montgomery County and boasts an employment rate for participants of over 80% and a high school graduation rate of 95%.  CTP provides a variety of services including career assessment, supported employment services, supported educational services as well as a variety of case management services.  The program is unique as it works one on one with students in the community to assist with goals that students themselves create.  CTP was created in 1993 as a partnership between St. Luke’s House and the Transition Unit of Montgomery County Public Schools (MCPS).

CTP is currently accepting referrals and is actively recruiting Montgomery County juniors, seniors, recent graduates, GED students, and students that have recently exited high school that are between the ages of 16 and 22 and have mental health concerns. The mental health diagnosis can be documented by a clinician i.e. depression, ADHD, Bipolar Disorder, Anxiety Disorder, etc. or have had a  school IEP coding documenting an Emotional Disability to be eligible for the program.  We have an open referral process - and students can self refer, or parents, school, community services, private providers, faith communities, etc. can refer.

 

The Career Transition Program (CTP) is now able to serve many more youth than in the past, through funding provided by participation in the Social Security Administration’s Youth Transition Demonstration Project (YTD) .The Youth Transition Demonstration Project is a research study funded by the Social Security Administration. Program services are provided at no cost to the students or their families.  All applicants participate in a random assignment with half of the applicants being selected for CTP services.  Students randomized to the control group are eligible receive a technical assistance session from CTP staff at which time we will also provide the student with a resource manual to help guide them in their transition process. 

 

Just for signing up, the youth receives a $10 Target gift card.
