Rehabilitation Coordinator

Our Supported Living outreach program offers psychiatric rehabilitation services to people who are living in the community. Services can be provided in their own homes, family homes, or elsewhere in the community. This individualized service has helped thousand of individuals struggling with mental illness to successfully manage their illness, housing and their ability to maintain being an integral member of society. Life goals and dreams which otherwise may not have been obtainable, are now obtainable due to community based Supported Living services.

This program currently has a need for Rehabilitation Coordinators. Within this position you would be given the following responsibilities:


Work with approximately 15 clients with Axis 1 and Axis 11 disorders.


The Rehabilitation Coordinator will plan, develop, coordinate and document all services received by their client caseload including the Vocational, Life Skills and Day Programs. They will act as an advocate, communicating their clients’ progress to both internal and external treatment teams and ensure adequate communication is continuing between all treatment teams.  The Rehabilitation Coordinator will also work to assess their clients’ needs for living in the community and thus develop goals in collaboration with their clients.


To qualify for this position you need to have a Bachelor’s degree in a related field such as; psychology, sociology, rehabilitation counseling or social work. Related experience may be substituted for a degree; however related experience must represent a total of at least four years. Previous experience working in mental health, is preferred but not required.


Candidates need to have full access to a vehicle, with no more than 2 points on your driving record, as the position requires significant travel throughout Montgomery County.


You must also be willing to work some evenings and weekends, meeting with clients in the community or in their own homes. Work hours may vary, allowing for a flexible schedule. To learn more about this position and our agency, please visit our website at: www.stlukeshouse.org.
