St. Luke’s House’s Supported Living outreach program offers psychiatric rehabilitation services to people who are living in the community. Services can be provided in their own homes, family homes, or elsewhere in the community. This individualized service has helped thousand of individuals living with serious and persistent mental illness to manage their illness, their housing and their ability to maintain being an integral member of society. Life goals and dreams which otherwise may not have been obtainable, are now obtainable due to community-based Supported Living services.
If you are interested in helping people improve their quality of life in their community, you may apply for an internship or a volunteer position within our Supported Living Program, as a rehabilitation coordinator. In this position you would be given the following responsibilities:
• Plan, develop, coordinate and document all services received by your client caseload including from the Life Skills Program, vocational, and day programs.
• Act as an advocate, communicating the clients’ progress to both internal and external treatment teams and ensuring adequate communication continues between all treatment teams.
• Assess clients’ needs for living in the community and then develop goals in collaboration with those clients.
Candidates need to have their own vehicle and no more than 2 points on their driving record, as their responsibilities often include transporting clients.
St. Luke’s House offers excellent training, mileage reimbursement, cell phone stipend, flexible schedule and, for those requiring it, guaranteed licensed clinical supervision.
